The online training includes:
Phase 1. Online modules, live webinar, and resources to learn all about CGM
Phase 2. Implementation support for prescribing providers looking to apply their knowledge to practice
□ Target Audience: Any healthcare provider (e.g., nurse practitioner, physician, physician assistant, pharmacist) who manages and prescribes to individuals with the diagnosis of type 1 or type 2 diabetes
□ Project Design: A DNP Project with a Quality Improvement Design to help you manage your diabetes patients. Training and Implementation support are provided for participation by a diabetes care and education specialist.
□ Free, three hours of on-line training at your own pace. One-hour live interactive webinar with the instructor.
□ Where: All Online from the comfort of your computer at home, work, or anywhere you have internet access.
